The Challenge
The challenge
A large NHS Trust was running a patient referral system built in the early 2000s. The system was slow, difficult to use, and required manual workarounds from clinical and administrative staff. Referral processing times were running at an average of 8 days — far above the target of 5 days.
The system was also completely inaccessible to users with disabilities, creating compliance and equality risks for the Trust.

We ran a six-week discovery phase with clinical staff, administrators, and patients to understand the current-state journey in detail. This surfaced a number of pain points that weren't visible in the original brief.
We then designed and built a new cloud-hosted referral platform on AWS, integrated with the Trust's existing EPR system via HL7 FHIR APIs. The new system was designed to GDS standards and achieved full WCAG 2.2 AA compliance throughout.
The project was delivered in four agile sprints over 16 weeks, with a phased rollout to minimise disruption to clinical operations.

Average referral processing time dropped from 8 days to 4.8 days — a 40% improvement and ahead of the Trust's 5-day target. Administrative staff reported a significant reduction in manual workarounds and the new system passed the Trust's internal WCAG accessibility audit with zero critical issues.
The system has since been adopted by two additional NHS Trusts within the same Integrated Care System.
